EZ Loader v2.60 by David Schmoll (Released April 25th, 1994) is probably one of the nicest menu systems I have seen for the Commodore 64 /128.  This program has tons of features and supports all kinds of devices including CMD hard drives, floppy drives and even TOD clocks to support auto loading a program at a specific time.  The documentation on the disk is very thurough and should answer any questions the built-in help doesn't.
